收集個人資料告示

當你提供個人資料予本中心時，請詳細閱讀本告示。

一、 收集個人資料的目的：

你所提供的個人資料，將會被西貢區社區中心(本中心)作為申請有關服務之用。提供個人資料予本中心是自願性的。若你未能提供足夠個人資料，本中心可能無法處理你的申請或提供有關服務。請確保你所提供之資料正確無誤，並請盡快通知本中心有關任何資料的改動。

二、 發放資料與受讓人的類別：

你所提供之個人資料，將會按需要而提供予本中心的職員或指定人士使用。此外，本中心亦會在有需要時披露有關資料予下列團體：
(1) 活動、課程或服務的資助/協辦機構或團體、承辦商或服務提供者。
(2) 已預先得到資料當事人的同意，或
(3) 遵循所有適用法津、規定、法律程序、具法律效力的政府要求、行政制度或規例要求。

三、 查閱及更正個人資料：

你有權要求查閱和改正所提供的個人資料及索取有關資料的複本。如需查詢或改正其個人資料，可填妥有關查閱個人資料要求表格，或以書面形式申請，並電郵至 skdcc@skdcc.org。本中心在處理查閱或改正資料要求時，會查核提出要求者的身份，以確保他／她在法律上有權作出這項要求。本中心將可能就此查閱或得到有關資料文件複本收取合理的費用。

四、 申請人聲明：

1) 本人明白及同意西貢區社區中心會透過本表格收集本人的個人資料，並利用我的個人資料，作處理申請、安排課程 / 活動 / 服務內容、宣傳、檢討服務、統計、意見調查、調查／跟進其他與西貢區社區中心有關的服務或事宜之用。
2) 本人明白及同意西貢區社區中心可能把本人的個人資料作上述的資料轉移用途。
3) 本人明白及同意西貢區社區中心可能會使用本人的個人資料（包括但不限於姓名、地址、電郵地址、電話號碼），向本人提供有關西貢區社區中心的相關活動、課程或服務及推廣活動等的相關資訊。

五、 查詢：

Personal Information Collection Notice

When you provide your personal data to the “Center”, please read this notice carefully.

1. Purposes of collecting personal data:

The personal data you provide will be used by Sai Kung District Community Centre(the Centre) for the purpose of processing relevant service applications. Providing of your personal data to the Centre is not-obligatory but failure to provide sufficient personal data may result in the Centre being unable to process your application or provide the relevant services. Please ensure that the information you provide is accurate and notify the Centre for any changes to the made promptly.

2. Disclosure of personal data and Categories of data recipients:

The personal data you provide will be accessed by the staff of the Centre or designated personnel as necessary. In addition, the Centre may disclose relevant data to the following entities when necessary:
(1) Sponsoring/co-organizing organizations ,contractors, or service providers for activities, courses, or services.
(2) With the prior consent of the data subject, or
(3) In compliance with all applicable laws, regulations, legal proceedings, legally binding government’s inquiry, administrative systems, or regulatory requirements.

3. Access and correction of personal data:

You have the right to request access to and correction of your personal data provided and to obtain a copy of the relevant data. If you wish to do so, please fill out the Personal Data Access Request Form or submit a written application via email to skdcc@skdcc.org. For protection of your personal data, the Centre will verify your identity when processing of such data access or correction request. The Centre may charge a reasonable fee for processing of such data access, correction request or provision of copies of the relevant data documents.

4. Applicant's declaration:

I understand and agree that Sai Kung District Community Centre will collect my personal data through this form and use my personal data for processing applications, arranging course/activity/service details, promotion, service review, statistics, opinion surveys, research/follow-up on other services or matters related to Sai Kung District Community Centre.
I understand and agree that Sai Kung District Community Centre may transfer my personal data for the aforementioned purposes.
I understand and agree that Sai Kung District Community Centre may use my personal data (including but not limited to name, address, email address, phone number) to provide me with information related to activities, courses, services as well as promotional activities of Sai Kung District Community Centre.
